Pediatric Case Management
This service is for families with children, newborn to seventeen years of age, navigating serious or complex health conditions or experiencing an acute health event. Our certified nurse case managers support families and caregivers through various care coordination needs including medical care as well as social, emotional, and development services and supports. Adults with Special Needs Case Management
If you’re caring for an adult with an intellectual or developmental disability and working to navigate their medical health needs, this service is for you. Our experienced nurse case managers are available to support with consultation, medical care coordination, and communication management among treatment providers, social workers, and families.
Geriatric/Elderly Case Management
Our nurses provide a holistic and client-centered approach to managing medical care for older adults facing ongoing health care needs. Our certified nurse case managers assist fiduciaries and families through consultation, a comprehensive nursing assessment, care planning and coordination, and advocacy-based case management.
