Tracy leads the nurse case management team with over 25 years of nursing experience in acute and outpatient settings, case management, medical sales, home infusion and enteral therapy, oncology and hematology, urology and more. She’s been a panel expert at the Massachusetts State Health Policy Commission Convening on her transformation program to reduce unnecessary hospital utilization. 

Tracy has been the recipient of the Presidential award by Baystate Health System for team collaboration across the health care continuum as well as a distinguished performer award for reducing patient barriers to treatment and continuity of care among accountable care organizations.
